• H & R LLC dba On The Hook Fish and Chips, North Salt Lake, Utah — mobile food vendor
• Johansen LLC, Las Vegas — salvage removal
• NV Extreme Builders, Reno — carpentry contractor
• Alpine Basin Care, Fernley — medical office
